Graduate, Achievements and Scholarship Application - 2024
MPBC Youth, Parents| Guardians, and Adults:
It is that time of year again where we recognize our MPBC Youth and Adults that will be graduating, received achievements or awards and or applying for the MPBC Scholarship.
You must be a Disciple of Mount Peace to complete any portion of this Application!
The Recognitions are scheduled for Sunday, June 9, 2024.
The registration form is set up in four (4) sections:
Section A - Applicant Data
All applicants should submit your High School or College Graduate picture in your cap and gown or a school picture. The picture should be in jpeg format (at least 600 dpi).
Section B - Graduate Information
If you are a High School, College, Technical School or University Graduate, please complete Sections A&B.
Section C - MPBC Scholarship Application
If you plan to apply for the MPBC Scholarship, please complete Sections A&C.
Section D - Adult or Youth Achievements and/or Awards
If you have achievements or awards, you would like recognized, please complete Sections A&D.
Please list no more than THREE (3) of your major accomplishments or awards.
All submissions are due into the Administrative Office on | or before May 26, 2024.
